5.1 Trademarks Claimed

Ultralite Enterprises, Inc. makes claim to the trademarks below.

5.1.1 ULTRALITE

Ultralite Enterprises, Inc. makes the claim to the name ULTRALITETM and company logo (Figure 1).

5.1.2 NSITE

Ultralite Enterprises, Inc. makes the claim to the phototherapy solution/application name NSITETM, solution logo (Figure 2), and solution/application icon (Figure 3).

5.2 Trademarks Notifications

Notification of the claim for trademarks must be presented in documentation and materials disclosed outside of the company. This includes, but not limited to, identifying the claims in Ultralite owned and/or operated websites, company and product brochures/presentations/videos, user documentation, and any other form of paper and electronic communications in which is presented outside of the company.

The trademark notification must be presented at the first appearance of each trademarked name within the published communications.

For example:

The ULTRALITE† Professional Full-body Phototherapy System with the 6809 Phototherapy Controller can be used within the NSITE†† Intelligent Phototherapy Management Solution.”

Then provide footer claim statement:

† ULTRALITE and the ULTRALITE logo are trademarks of Ultralite Enterprises, Inc.
†† NSITE and the NSITE logo and NSITE icon are trademarks of Ultralite Enterprises, Inc.

The following is Ultralite’s privacy policy which is applicable to all Ultralite owned/operated websites and other areas in which we collect personally identifiable information. This statement must be presented verbatim in all customer-facing documents, as appropriate (e.g. user manuals, website, etc.):

Ultralite Enterprises, Inc. (“Ultralite”) respects the privacy of our website visitors and manages our websites
accordingly. The protection of your privacy and personal data is an important matter for us. To ensure a high level of transparency and security, our privacy policy as set forth below will inform you about the nature, scope and purpose of any collection, use and processing of personal data recorded by us.

This privacy policy applies to a website (the “Website”) that is either (a) owned and operated by Ultralite or (b) includes content, including but not limited to registration and other forms, owned by Ultralite or drawn from a website owned and operated by Ultralite.

By using the Website and providing personally identifiable information about yourself, you do hereby consent to (1) our Terms of Use, (2) our collection, use, and sharing of data as described in this Privacy Policy; (3) the notice, choice, access, and security we are providing, as described herein; and (4) the methods we have described to notify you of changes to this Privacy Policy.

What Information We Collect and How We Use It
Users of the Website may browse without being obligated to submit any personal information. During your visit, the web servers only collect basic information such as referring page, file requested, date/time, and IP address.

In certain areas of the Website and for specific purposes, we may collect certain personally identifiable
information from you. These purposes include:

  • providing personal contact information to us so that we may contact you in the ordinary course of

  • providing Ultralite's newsletter to you via e-mail;

Testimonials
We greatly appreciate the testimonials that our customers provide and we’re proud to show our testimonials.

We post customer testimonials to a variety of Ultralite-owned sources which may include the Website,
marketing brochures, and software applications. The posted testimonials often include personally identifiable information such as the customer's name and organization.

If you want to remove your personal information that we display, please send your request to
support@ultraliteuv.com.

Children’s Privacy
Ultralite does not solicit personal information from children. We abide by the United States Children's Online Privacy Protection Act and will not knowingly collect personally identifiable information from children under age 13. We recommend that parents actively participate in their children's use of the Internet.

Choices You Can Make About Your Information
When subscribed to the Ultralite Newsletter, you may opt-out of receiving any further e-mail from Ultralite by clicking on the "unsubscribe" button in any marketing email.

By adjusting the settings in your browser, you can refuse to accept cookies or give yourself the opportunity to
accept or reject cookies, or receive notice of cookies. Software programs are available that block cookies.
Refusing cookies may impinge or block altogether your user experience on the Website by eliminating
